What Is a Smart Lens?

Smart lenses are multifocal artificial lenses placed inside the eye. They offer a permanent solution for high prescriptions where laser isn't suitable, or for age-related loss of near vision (presbyopia); at the same time, they prevent/treat cataracts.

What is a Smart Lens (EVO ICL), and how does it work?

The ICL (Implantable Collamer Lens / EVO ICL) is a thin lens placed inside the eye without touching the cornea. It's fixed in the space between the iris and the lens (the sulcus); it provides refractive correction without affecting intraocular pressure or the natural lens.

When is a smart lens preferred over LASIK?

ICL becomes the primary choice if the cornea is very thin or irregular (including suspected keratoconus), if the prescription is very high (myopia over -6.00 D), if dry eye syndrome is present, or if the patient is wary of the irreversible nature of LASIK.

Is the ICL reversible?

Yes. One of the most important advantages of the EVO ICL is its reversibility. If a cataract develops in the future, or the lens needs to be removed for any reason, the procedure can be reversed.

Older ICL models were associated with increased intraocular pressure and cataract risk. The central hole in the EVO ICL (KS-Aquaport) allows free circulation of aqueous humor; this new design has largely eliminated the risk of cataracts and pressure.

Vision improves markedly on the day of the procedure. Near-complete clarity is achieved within 24–48 hours. Antibiotic + anti-inflammatory drops are used for 1 week. Check-ups are done at week 1 and month 1; no special restrictions are needed beyond the eye drops.

The EVO ICL has very little effect on night vision. While high myopia itself causes halos and starbursts at night, these symptoms usually decrease or disappear after ICL.

The cost of ICL for both eyes, which is €3,000–6,000 in Europe, is offered much more accessibly in Istanbul, including accommodation. All pre- and post-operative check-ups are included in the package.

The lower age limit is 21, and the upper limit is usually set at 45–50. Over 50, whether a cataract is beginning is evaluated; where a cataract is the more practical solution, an IOL is recommended.

The standard approach is to treat the eyes 1 week apart. This protocol allows the desired outcome to be assessed after the first eye before planning the second, and keeps any potential complications limited to one eye.

EVO ICL lens sizing is done precisely before surgery, and lens subluxation (displacement) is extremely rare. A deliberate design change has minimized cataract risk. All of these risks are shared clearly with you during the pre-examination.
